This issue appears when LMS accessed from Internet Explorer Because Internet Explorer's compatibility mode causes problems with the LMS interface.

Resolution:

Turn compatibility mode off, or remove the LMS website from the compatibility mode list in the Internet Explorer compatibility view settings.
